Health inspections

Aug. 31

LT Ball Intermediate School, 575 N. Hyatt St., Tipp City — No violations were documented at the time of inspection.

Subway, 998 N. Market St., Troy — Repeat: the FSO did not have a person in charge that had completed a Level One Certification course. Repeat: facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. Critical, repeat: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Repeat: non-food contact surfaces of equipment are dirty. Observed food residual on the handles of the bread oven and the walk-in cooler.

Sept. 1

Leaf & Vine, 108 W. Main St., Troy — Critical: the person in charge at time of inspection was not certified in food safety. Observed unclean residual in the bottom of keg cooler and small two-door cooler. Repeat: women’s toilet room doors are not being kept closed when required.

The Golden Leaf Tea and Herb Co. Ltd, 128 E. Main St., Tipp City — Critical: the following food items were observed without a date marking: small package of deli ham, small package of deli turkey. Critical: at the time of inspection, the following food items were being used past their use-by-date: small container of chicken salad (8/23), small container of egg salad (8/16), medium container of Devonshire cream (8/18).

Little Caesars, 1408 W. Main St., Troy — Critical: person-in-charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Repeat: person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. Observed non-handled utensil being used to scoop out flour out of large container underneath dough press machine. Observed no food labels on the containers holding flour underneath dough press machine. Observed that the sanitizing test strips used in the ware washing area are completely ruined, and the cases are covered in a dark residual. Observed handle of the hand-drying dispenser not working properly above hand washing sink towards the front of the food prep area. Handle kept getting stuck when pushed in. Observed ice accumulation on the top and around the door frame of the reach-in freezer across from the pizza oven. Critical: observed food residual build-up on the inside of the dough press machine. Repeat: observed residual build-up on handles of the hand drying dispenser, handle of the walk-in cooler, handle and hinge on the dough mixer, dust on the fan cases of the condensing unit in walk-in cooler, debris build up on the metal racks in middle of food prep area, residual build-up on the sinks inside of ware washing area, dark residual on the racks in storage areas and walk-in cooler, dust accumulation around the pizza oven and the ceiling tile above, residual build-up on restroom door handle and restroom light switch, residual on vice grip on table by walk-in cooler door, residual on gasket on walk-in cooler door. Observed dark residual build-up along the top of the drain below the food prep sink.

Steak N Shake, 1779 W. Main St., Troy — Observed soil and food residual build-up on the sprayer of the sink in ware washing area. Repeat: non-food contact surfaces of equipment are dirty. Observed soil and food residual build-up on the handles of the sinks in the ware washing area. Observed that the lighting inside of the walk-in cooler was less than 50 foot candles (27 FC). Repeat: the physical facilities are not being maintained in good repair. Observed ice accumulation on the pipes of the condenser unit in the walk-in freezer. Repeat: the physical facilities are not being maintained in good repair. Observed condensation on the ceiling of the walk-in freezer.

Sept. 5

Bob Evans Farms, 1749 W. Main St., Troy — Critical: in the reach-in coolers and freezers on the cooking line, containers of food were observed without lids or coverings. The hot water sanitizing ware washing machine sanitizing temperature measure device was observed reading 171 F while the temperature measuring strip indicated the water temperature was at or above 180 F. Critical: in the back kitchen area, the deli slicer was observed with food debris build-up. Critical: in the front kitchen area, and then in the dish area, clean utensils and plates were observed with food debris build-up. The following non-food contact surfaces were observed with food debris build-up: in the front kitchen area, the bottom shelves, the pan drying shelves, the surfaces beneath the hard-top grills, the door gaskets of the non-functioning coolers beneath the hard top grill, the shelving units in the walk-in cooler, the shelving units in the dry storage room. Utensil sorters and lid storage containers in both the front and back kitchen areas were observed with food debris build-up. Throughout the back kitchen area, inadequate amounts of grout were observed between the floor tiles.
