Speedway, 1551 Covington Ave., Piqua — Provide thin-tipped metal stem thermometer so thin foods can be properly monitored. The pop machine is leaking. Repair. The following items need to be cleaned: single service drawers, tongs, holder on hot dog roller, counters. Tongs for dispensing food only being washed three times a day. These items must be washed every three hours of continual use to prevent bacterial growth.

Speedway, 1475 W. Market St., Troy — The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. Critical violation: handwashing sink is being used for purposes other than handwashing. Observed automatic soap dispensor not properly at the handwashing sink(s). Observed a unclean areas in hard to reach and hard to clean areas.
Arbogast Buick Pontiac GMC, 3540 S. Co. Rd. 25-A, Troy — Operation is satisfactory. Discussed regulations changes effective March 1, 2016.
Caldwell House, 2900 Corporate Dr., Troy — Non-food contact surfaces of equipment are dirty. Cart next to prep table has debris on all shelves. Rubbermaid trash can has grime and debris build up. The physical facilities are not being maintained in good repair. An area of the wall by the reach-in freezer had been cut out. Paint is chipping on multiple surfaces (door frames, areas of the wall.) Cabinet surface cover pulling away from particle board. Cove base behind Rubbermaid trash can has pulled from the wall. Observed a build-up of dirt and debris on the following: hand sink; wall by hand sink and dish machine; floors behind ice machine and under shelves in dry storage area, ceiling above cabinet by ice machine, all exterior and interior of cabinets and drawers and inside light shields in dry storage.
Clark’s Pizzeria, 420 S. Miami St., West Milton — Operation in good condition at time of inspection.
Cold Stone Creamery, 1187 Experiment Farm Rd., Troy — Non-food contact surfaces of both paper towel holders and vents on the two door reach-in freezer in the front area have accumulation of dust. Observed a build-up of dirt and debris on the shelves below the ice cream display counter. Walk-in freezer floor is buckling. Floor must be sealed, smooth and easily cleanable. May want to consider replacing floor.
Hong Kong Chinese Restaurant, 14 S. Tippecanoe Dr., Tipp City — Critical violation: observed containers holding food that are unclean with old food residual. Observed food in contact with an unclean surface or linens. Observed improper use and/or maintenance of wiping cloths. Time as a public health control was being used without the proper written procedures. Critical violation: observed unmarked time on food that was being used with time as a public health control for up to four hours. Example is the sauces made in the food service operation. The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized. Critical violation: hand washing sink is being used for purposes other than hand washing. Observed improper draining of condensation in walk-in cooler into bucket. Observed light bulbs or heat lamp that were not properly shielded or coated where required. Observed walk-in cooler with card board on floor, Walk-in cooler must be resurfaced with new smooth and easily cleanable surface. The physical facilities are not being maintained in good repair.
55 Express, 503 E. Staunton Rd., Troy — Observed sleeping and living area in walk-in cooler and items that assist a person in living and sleeping in the building. The person in charge did not ensure that FSO or RFE operations were not conducted in a private home or in a room used as living or sleeping quarters. Immediately remove all things that assist a person in living in this building. Observed cup utensils in box on floor. Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Observed two water leak from plumbing sources. The plumbing system does not comply with the Ohio building code. Observed no towels or hand drying device at the hand washing sink. Clean all hard to clean areas of build-up. Clean restroom and back room also. Observed a build-up of dirt and debris.

Coopers Farm Catering & Banquet, 3155 St. Rt. 48, Ludlow Falls — Satisfactory at time of inspection.
Residence Inn, 87 Troy Town Rd., Troy — Critical violation: TCS foods were not being held at the proper temperature. Shredded cheese at 51 degrees. Time as a public health control was being used without the proper written procedures. Shredded cheese and sour cream at self-service area. Critical violation: observed improper use of time as a public health control for up to four hours. Ensure the cheese and sour cream are marked with the time once removed from refrigeration. Single-use articles are not properly stored. Store eating utensils in self-service area with the handles facing the same direction to help prevent hand contact with mouth contact surface of utensil. Observed mops being dried using an unapproved method. Mop was laying on bucket in mop sink.
Taco Bell, 1420 W. Main St., Troy — Good Job! Facility was clean and in good order. Person-in-charge knowledgeable, temperatures within required ranges, observed good hand washing habits and glove usage.
Fletcher Pizza Carry Out, 101 W. Main St., Fletcher — Non-food contact surfaces of equipment are unclean, including samples cooler, refrigerator, cooking equipment, and sinks. Floor needs resurfaced. The physical facilities are not being maintained in good repair. Observed a build-up in hard to reach areas throughout the food area. Observed unnecessary or nonfunctional items and on the premises.
DJ’s Pizza Alley, 18 S. Second St., Tipp City — Critical violation: observed raw chicken over cut potatoes; this food was not properly protected from contamination by separation. Freezer is not approved by a recognized testing agency. Observed chicken cooler and freezers with unclean residual on surfaces. Non-food contact surfaces of equipment are unclean. Observed improper draining of condensation in walk-in cooler.
Meijer, 1900 W. Main St., Troy — Observed cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. They were observed next to the hand sink subject to splash. Corrected during inspection. The plumbing system was not properly repaired. Observed a leak under the prep sink in the produce area.
